Industrial Training Certificate for B.Pharmacy (if mandatory as per University Regulations)

Residential proof (Election ID/Aadhar card/Bank account/Indian Passport/ ration card/driving license)

8.

Employment proof Certificate ( Drug License/College/Industry ID etc. copies)

Fees Payment ( Rs.560/- by Cash)

Recent 5 Passport size color photos

DD for confirmation of Degree from respective University (if applicable)

Fill the application and tag all the documents in sequence as per CHECKLIST
|
IV.
|
Submit application IN PERSON in the office of Andhra Pradesh Pharmacy Council on your TOKEN DATE.

V.
|
Get Originals endorsed & stamped to complete application process.
